If you are a new client, please read the first two documents below. They explain in detail the policies and procedures you are agreeing to as a client of the Nashville Christian Counseling Center (NC3), as well as your rights according to the Health Insurance Portability and Accountability Act (HIPAA).

These documents will remain available on the website for you to reference or print at a later time if you would like. Please contact your therapist directly about any questions you may have regarding your responsibilities and/or rights as a client.

The "Initial Paperwork" document is to be completed and brought to your first session. If you do not have access to a printer, please notify your therapist and s/he will leave a paper copy for you to complete in the waiting room. To avoid the unnecessary use of your therapy hour in such cases, clients are encouraged to arrive at least 20 minutes prior to their initial appointment to complete the paperwork.
